Survey Reward Voucher — Terms & Conditions

These terms govern the free splint voucher offered by ODL Sleep Services ('ODL LABS') ("we", "us", "our") to clients who complete our Client Forecast Survey.

The offer

Eligible clients who complete the Client Forecast Survey will receive a voucher for one (1) free Full Coverage Nylon Splint, redeemable in our online dental store. The voucher is delivered by email shortly after the survey is submitted.

Eligibility

The offer is open to dental professionals and practices that are clients of ODL Sleep Services ('ODL LABS') (for example dentists, orthodontists, dental hygienists and practice managers). It is intended for business (B2B) recipients and is not open to the general public.

How to claim

1. Complete the Client Forecast Survey using the link in our invitation email.
2. A unique voucher code will be emailed to the address you provide in the survey.
3. Add the Full Coverage Nylon Splint to your cart in our online store and enter the code at checkout.

One voucher per person and per practice

• Each email address may complete the survey and receive a voucher once.
• We also limit the offer to one voucher per practice. Where several team members from the same practice respond, only the first valid entry for that practice will receive a voucher.
• Voucher codes are unique and single-use.

Redemption

• Each voucher entitles the holder to one (1) Full Coverage Nylon Splint at no charge.
• The voucher can only be used in the ODL LABS online dental store and only against the Full Coverage Nylon Splint.
• Vouchers are not transferable, cannot be exchanged for cash or credit, and cannot be combined with other offers.
• Standard product, ordering and delivery terms for our online store apply to any order placed using a voucher.

Validity

Vouchers are valid until 30 September 2026. Codes cannot be redeemed after this date.

Use of survey responses

Survey responses are used to help us plan our lab capacity, stock and services. We handle your information in line with our Privacy Policy. Completing the survey does not create any obligation to refer patients to us or to place orders with us.

General

We may amend, suspend or withdraw this offer, or these terms, at any time. We may also withhold or cancel a voucher where we reasonably believe the offer has been misused (for example duplicate or fraudulent entries). These terms are governed by the laws of Australia.
